The more eligibility requirements you have, the greater discounts and scholarships you are eligible for. You can submit a picture or screenshot in your submission but AOTG has the right to request the official documentation, if needed.

For this option, include paystubs or other payroll information that proves you are low-income. This can include unemployment or worker's compensation. Also, include lawsuit judgements for worker's compensation claims.

Government Assisted Program *

Government Assisted Program documents of approval or grant amounts. All documents must be dated within 30 days - 90 days depending on the program. Documents can also include determination letters, monthly snap reports records, etc.

18 Years Old or Younger *

Can use a school identification, depending on child's age. Birth certifcate or Certificate of Acknowledgement Notorized.

Other Identified Minority *
